Please complete this form by August 1st for the Fall Semester and by December 1st for the Spring Semester.  Every effort will be made to accommodate your scheduling requests.  You will receive the Fall Participation Calendar at your Orientation Appointment and the Spring Calendar before Winter Break.  

If you have a conflict with your scheduled days, please work out a trade with another parent or obtain a Paid Parent Participator.  If you use a Paid Parent Participator or if you are a no show, you will be charged a $40 fee.  A list of Paid Parent Participators is available.
